In line with Transforming Your Care proposals, a total of seventeen Integrated Care Partnerships (ICPs) have been created across Northern Ireland in the following areas:
Belfast ICP Area – North, South, East and West
Northern ICP Area – East Antrim, Antrim,/Ballymena, Mid Ulster and Causeway
South Eastern ICP Area – Lisburn, North Down, Ards and Down
Southern ICP Area – Newry/Mourne, Armagh/Dungannon, Craigavon and Banbridge
Western ICP Area – Northern Sector and Southern Sector
ICPs enable closer working between and within hospital and community services and in the first instance, ICPs will focus on FREDS – Frail elderly, Respiratory, End of Life, Diabetes and Stroke.
Alongside GP’s and representatives from the voluntary and community sector, two places in each partnership have been allocated to pharmacists. Click here for more information.
